For independent contractors (not working for someone else) or businesses operating as massage parlors you must obtain both a business permit and an employee identification card and pay business tax. (Read Scenario #1)
If you are an employee of a business who already has a valid business permit, you only need to obtain an employee identification card. (Read Scenario #2)
Scenario #1 - If you need both a business permit & an employee identification card:
Step 1 - Contact the Police Department at (310) 285-2184 to make an appointment to be fingerprinted and photographed.
Step 2 - Before you can be fingerprinted and photographed you must complete a Regulatory Permit Application, Finger Print Form, and the City of Beverly Hills Applicant Background Form. These applications will be signed by the applicant under penalty of perjury and must be submitted in person. The Regulatory Permit Application will be submitted to the Cashiers’ Office located at 455 N. Rexford Drive, Beverly Hills, CA 90210. The application processing fees are adjusted annually. The Fiscal Year 2009/10 fees are:

Step 3 – Submit your completed Applicant Background Form and Request for Live Scan Service Form to the Beverly Hills Police station (Records Section, Third Floor). At this point you will be fingerprinted, photographed and your background check will begin. You will be notified by mail of your approval status within 3-4 weeks.
Step 4 - If your permit is approved, a second set of fees is required. The Fiscal Year fees 2009/10 are:

These fees can be paid for over the phone or in person. Once the second set of fees has been paid your business permit and employee identification card will be mailed to you.
Scenario #2 - Employees of a business that is currently permitted as a Massage Parlor:
The following is for an employee or independent contractor of a business that is currently permitted as a Massage Parlor within the City of Beverly Hills.
Step 1 - Contact the Police Department at (310) 285-2184 to make an appointment to be fingerprinted and photographed.
Step 2 - Before you can be fingerprinted and photographed you must complete the Finger Print Form and the City of Beverly Hills Applicant Background Form. These applications will be signed by the applicant under penalty of perjury and must be submitted in person to the Police Department located at 464 N. Rexford Drive, Beverly Hills, CA 90210. The Police Department phone number is (310) 285-2184. You must also provide a letter on Company letterhead from the business stating you will be working as an employee of the business and that the business will pay your business tax. You must also provide valid picture identification or evidence of non-citizen immigration status.

Step 3 – At this point you will be fingerprinted, photographed and your background check will begin. You will be notified by phone of your approval status within 3-4 weeks.
