Long Term Care

The City offers a long term care program for employees and family members through UNUM. The cost of Long Term Care depends upon your age, the amount of years you want to be covered for, the amount of coverage you want each month, and if you want inflation protection (optional).

Spouses, Registered Domestic Partners and all family members can apply for Long Term Care. They must complete the Benefit Election Form, the Long Term Care Insurance Application (medical questionnaire) and must be approved for coverage in order to enroll in the program.

New employees may sign up for Long Term Care when they first begin employment with the City. Should they miss their eligibility period, they can enroll during open enrollment or at any time. If they enroll at any time other than the eligibility period or open enrollment they will have to fill out the Long Term Care application.
