Address Changes

• In the case of an address change, the property owner or an agent thereof should submit a written request, plot plan and floor plan of the property, and payment of $588.70.

Address Verifications

• In the case of address verification by a tenant, please complete the Tenant Address Verification Form.
• In the case of address verification by a property manager, please complete the Property Manager Address Verification Form.
