  1. City of Beverly Hills Engineering Department shall be named as Additional Insured.
  2. City of Beverly Hills shall be named as the Certificate Holder.
  3. Insurance policy must be valid at the time of permit issuance.
  4. Insurance policy must remain valid throughout the permitted work period.
  5. Insurance shall be provided by the licensed general contractor.
