Passport Services

Passport services are only offered by appointment.  There is no walk-in service.

Please take a moment to familiarize yourself with all the necessary requirements before signing up for a passport appointment. 

List of Requirements

Adults (applicants 18 and over):
  1. Proof of citizenship (for each applicant): most recent undamaged U.S. Passport or, if Passport cannot be provided, Original US Birth Certificate, Certificate of Naturalization, Consular Report of Birth Abroad, or Request for a File Search. The document must be an original or certified copy.
  2. Valid identification (for each applicant): valid California driver’s license, valid Passport, or Certificate of Naturalization. The document must be an original. If these documents cannot be provided, secondary identifications are required, such as valid government issued ID cards.  Please refer to travel.state.gov for more information.
  3. DS-11 form completed (for each applicant) in black ink and not signed. The applicant’s social security number is required on the form. The DS-11 is available at the Beverly Hills Public Library or can be downloaded from travel.state.gov.
  4. If your most recent U.S. Passport was lost/stolen, the DS-64 form must be completed, signed, and submitted along with your application. If you have already reported your passport lost/stolen online, please bring the printed receipt to your appointment.
  5. A check or money order made payable to the U.S. Department of Stateone for each applicant:
  6. There is a separate, federally mandated facility fee of $35.00 per application, payable by credit card only, to the Beverly Hills Public Library.
  7. A new passport photo is required for each applicant. Applicants may provide their own photos, or have their photos taken at our passport office. Please visit the Department of State website at travel.state.gov for a guide to photo requirements.
Teens (applicants ages 16 - 17):
  1. It is suggested that at least one parent/guardian appears along with the applicant. Otherwise, applicants must show proof of parental awareness, as described on travel.state.gov
  2. The parent/guardian is required to bring valid identification: valid California driver’s license, valid Passport, or Certificate of Naturalization. The document must be an original. If these documents cannot be provided, secondary identifications are required, such as valid government issued ID cards.  Please refer to travel.state.gov for more information.
  3. If available, the applicant(s) should bring valid identification.
  4. (Each) applicant’s proof of citizenship: most recent undamaged U.S. Passport or, if Passport cannot be provided, Original US Birth Certificate, Certificate of Naturalization, Consular Report of Birth Abroad, or Request for a File Search. The document must be an original or certified copy.

Children (applicants under the age of 16):
  1. The parents/guardians named on the birth certificate are required to appear along with the applicant(s).
    • If only one parent can attend the appointment, the non-appearing parent must submit a DS-3053 form, completed and notarized, as well as a front and back copy of the ID shown to the notary. One form and one photocopy is required for each applicant.
    • Or, if applicable, please bring custody decrees, court orders, death certificates, or other necessary custody documents. The documents must be original or certified copies.
  2. Parents/guardians must bring valid identification: valid California driver’s license, valid Passport, or Certificate of Naturalization. The document must be an original.  If these documents cannot be provided, secondary identifications are required, such as valid government issued ID cards. Please refer to travel.state.gov for more information.
  3. (Each) applicant’s proof of parental relationship, specifically (each) child’s Original US Birth Certificate, Consular Report of Birth Abroad, Foreign Birth Certificate and Translation, or Full & Final Adoption Decree. The document must be an original or certified copy. Hospital birth certificates and abstract birth certificates cannot be accepted.
  4. (Each) applicant’s proof of citizenship: most recent undamaged U.S. Passport or, if Passport cannot be provided, Original US Birth Certificate, Certificate of Naturalization, Consular Report of Birth Abroad, or Request for a File Search. The document must be an original or certified copy.

General Information

Please note: If you are eligible to renew your passport by mail using the DS-82 renew by mail form, but are choosing to have your passport processed by our Acceptance Facility, please be sure to complete the DS-11 form. We cannot accept the DS-82 renew by mail form.

Routine service - 6 to 8 weeks  

Expedited service - 2 to 3 weeks

Mail times may very. Please plan ahead and apply early.

Two hours of free parking is available in the parking garage north of the library. Validation is not required.

Appointment Reschedule/Cancellation: Links to reschedule or cancel your appointment can be found at the bottom of your confirmation email. Cancellations are automatically made available on the online appointment calendar. Additional Notes and Special Circumstances:

CITIZENSHIP THROUGH ADOPTION:

If you were born outside the United States and acquired citizenship through adoption by a U.S. citizen parent while under the age of 16, please submit the following:

  • Citizenship evidence of your U.S. citizen parent
  • Your full and final adoption decree
  • Evidence of lawful entry for permanent residence (Examples: Child’s foreign passport containing an ADIT stamp or Form I-94 with the ADIT stamp and picture)

NAME CHANGE:

  1. If you are changing your name, please provide documentation indicating the name change such as a Marriage Certificate, Divorce Decree, amended birth certificate, or court order stating the name change. Must be original document, photo copies cannot be accepted. For more information on name change requirements, please visit the State Department’s informational page at travel.state.gov.
  2. In the event you cannot legally document a name change (through court order or marriage), you must submit at least 3 public records showing your date and place of birth that show exclusive use of your assumed name for at least 5 years.

Limited Validity Passport

  1. In the event your most recently issued passport was limited in validity, please provide your limited validity passport in addition to a separate proof of citizenship, either an older passport or your Certificate of Live Birth as proof of citizenship; Must be original documents, copies or abstracts cannot be accepted. Obtaining birth records

  1. For information on obtaining a certified copy of a birth record from Los Angeles County, please visit LA County's Registrar-Recorder/County Clerk page.
  2. For information on obtaining a certified copy of a birth record outside of Los Angeles County, visit the Center for Disease Control’s informational page at cdc.gov.
